In homoeopathy, the choice of remedy is based on a consideration of the totality of an individual's symptoms and circumstances, including personality, behaviors, fears, responses to the physical environment, food preferences, philosophy of early life of patient, history of patient's nearest relatives and so on. No knowledge is perfect unless it includes an understanding of the origin - that is, the beginning; and as all man’s diseases originate in his constitution it is necessary that his constitution should be known if we wish to know his diseases.

Stuart close said - Constitution is that aggregate of hereditary characters, influenced more or less by environment, which determines the individual's reaction, successful or unsuccessful, to the stress of environment.

It is needed to assess the peculiarities of an individual during his or her apparently healthy state. The peculiarities of each individual are put together and assessed in three ways. They are-

• Actual constitutional diagnosis
• Development constitutional diagnosis
• Environmental constitutional diagnosis

Dr. Kent says, Physical constitution is the external disorder following disorder in the man, the vital force.

Constitution is a term which covers vast dimensions in width, depth and intricacy, but it is often inadequately understood. It has two basic factors-

• The endogenous factor
• The exogenous factor

The endogenous factors are innate in the individual incurred from the heredity. They endow the organism with various types of tendencies susceptibilities, reactions to the stress of environment.

The exogenous factors are incurred gradually and steadily since birth from the various intimate details of the environmental factors.

Ultimately, both these exogenous and endogenous factors may be come so much intimately mingled up that they form a new type of constitution of the particular organisms - totally distinct from progenitors and even others of the same species. From this approach it is clear that each individual has a distinctly characteristic constitution of his or his own.

It is for this reason necessary, that persons are studied in details in their distinctive features, that signify the personality. They bring out unfailing indications in the health-ill relations and guide the treatment.

Types of Constitution

There are different types of constitution which are carried into homeopathic parlance from old-time pathology. Some of them are-

• Scrofulous constitution: Glands remain swollen, general weakness, lack of reaction, wound and inflammations tardy to heal, considered to be the result of hereditary combination of Psora and Syphilis.

• Hydrogenoid constitution: Increased susceptibility to humidity, even to aquatic vegetables. Hence the symptoms of a disease in this constitution of the body are aggravated by every thing which in any way increases the atoms of water in the organism. Considered to be the result of intimate combination of Psora and Sycosis.

• Oxygenoid constitution: it is characterized by the accumulation of excess of oxygen with resultant destruction of body tissues. Body is thin and thus ill on change of season. Destructive, disorders involving the genitalia and diseases of central nervous system. Body is prone to ulcerations and self-destruction. Numerous white spots on the nails. 

• Carbo Nitrogenoid constitution: It is characterized by excess of carbon and nitrogen and suffers from functional disorders of body and mind, diminution of the absorption of oxygen by the tissues. Psora, eczema, arthritism, marked by erratic working or the heart, lungs, kidneys, liver, spleen  etc.

• Leucophlegmatic constitution: Catarrhal, flabby, water-logged constitution, with pale loose skin. Sluggish in all movements and activities. Chilly and susceptible to cold.

There are also choleric (bilious), chlorotic (anemic with greenish pallor), Neuropathic(Easily excited and depressed), Dyscratic, Lithemic, Neuropathic constitution etc.

Factors related to constitution

Constitution consists of various factors. The chief are-

• Physical make-up of the body
• Temperament
• Heat and cold relation
• Desire and aversion to food
• Miasm - PTS, PRS, Psora, Sycosis, Syphilis, Tubercular
• Diathesis
• Susceptibility and responses
• Addictions, habit etc.

It also includes all other factors that work in his framing and that make him distinct, definite and different and thus separate from other persons.

Therapeutic Importance

It is has been noticed that every individual drug has a special relative affinity for particular type of constitution. This fact was clearly noticed by Hahnemann while conducting drug proving as well as in his practice.

Constitutional Diagnosis

Constitutional diagnosis means the assessment of the peculiarities of an individual during his or her apparently healthy state.

Those that pertain to the individuality of the patient even when he is apparently healthy and which modify or qualify symptoms of the disease he is suffering from - helping us for person-diagnosis, i.e., the diagnosis of the constitutional individuality of the patient.

To understand this independently of his disease three things have to be investigated:

• Actual constitutional diagnosis.
• Developmental constitutional diagnosis.
• Environmental constitutional diagnosis.

The actual constitutional diagnosis is done by observing the peculiarities of the individual during his/her apparently healthy state. Only then, the physician clearly perceives what is to be cured in disease that is to say, in every individual case of disease.

The developmental constitutional diagnosis is done by examining and finding out the phases through which the patient has passed to reach his present actual psycho-somatic constitution of personality, including the hereditary investigation.

The environmental constitutional diagnosis consists in the assessment of the modalities or the individual's characteristic reaction e.g. how does this person react to his environment, as to time, place and circumstances.

Constitutional medicine

Role of Vital Force in Disease

As the vital force is spiritual, it can only be disturbed by some other spiritual force which is inimical to it. This inimical force affects the vital force when the former (inimical force) is superior to the latter (vital force). Because of this affection, the vital force no longer rules with unbounded away. Its normal workings are altered abnormally and the essential harmony is disturbed. This lack of harmony both on sensational and functional plane immediately shows itself through the material body as signs and symptoms. This is manifest in disease as it naturally developed because of disturbed vital force, and it develops also in disturbed vital force from the proving of remedies, showing the action of remedies and the very nature of the remedies themselves.....

In sec. 12 Hahnemann says- "It is the morbidly affected vital force alone that produce disease."

So, in health the immaterial vital force-

• Animates and controls the material organism without hindrance.
• Maintains the harmony of life producing normal sensations and functions of the body.
• Enables the reason gifted mind to engage in the higher purposes of life
• Resists the diseases and preserves the health.

Vital force dominates and controls the material organism and thus imparts life to it. The body moves, thinks, acts under the control of vital force. All our sensations and physiological functions, like respiration, circulation, digestion, excretion and immunity depend on it. Every manifestation of life solely depends on it.

Vital force co-ordinates in any orderly way the different physiological activities of life carried out by different organs of the body. His heart beats, lungs expand and contract, intestines carry on a peristaltic movement, etc. But a healthy man neither feels the existence of his organs, nor the functions carried out by them. In the state of health, every organ of body follows the will of man and the balance of mind and body maintained.

During the healthy condition of life, as there is a perfect balance between mind and body, the man can employ himself for the higher purpose of his existence.

In the healthy condition of man, the person protects himself from injurious influences. Our terrestrial existence are always exposed to thousands of disease forces but all of us do not always suffer because the vital force constantly affords protection against these Morbific forces and thereby keeps us in healthy condition.

Nature of Vital Force

Life is to be considered as composed of three parts - the body, the mind and the spirit. This is a trinity of life. The body and tissues are not life, but they are channels through which life functions.

It is this super-sensual thing which maintains the body in living condition. This makes all the difference between the living and dead. This thing, according to Hahnemann, the father of Homoeopathy is termed as Vital force - the force of life which appears first in the fifth edition of Organon (1833). In the sixth edition, the same is changed to "Vital Principle".

This was the first introduction to the medical world of the rational concept of life itself.

Nature of Vital Force

Vital force has the following characteristics or qualities-

• Spiritual
• Autocratic
• Automatic
• Dynamic
• Unintelligent and instinctive

Spiritual: This means immaterial. The vital force is immaterial and hence it is not perceptible to our senses. It is cognizable only by its effects when it works through the material body.

Autocratic: Auto means self, keratos means power. So autocracy means self-powered. The vital force is autocratic. It is the supreme authority in our body ruling in its own way. It is not governed by anything but governs the material body as the highest authority, so it is called autocratic. It remains in the organism as per its own will and maintains the integrity of the organism.

Automatic or self-acting: Vital force is an automatic force which animates the whole organism. It creates spontaneously to all stimuli against itself.

Dynamic: Vital force is dynamic i.e. only power or energy. This dynamism penetrates every particle, every cell, every atom of the human economy. Without this vital energy, the cell or the whole body becomes inanimate and is dead.

Unintelligent and instinctive: Hahnemann uses these denominations for vital force in Sec. 15, footnote of 22, 29 and in the introduction of Organon of Medicine.

15....(just as the vital force is not conceivable without the organism), consequently the two together constitute a unity, although in thought our mind separates this unity into two distinct conceptions for the sake of facilitating the comprehension of it.

Intellect means, "power to judge and comprehend." As it is only a force it can not reason out itself. Forces can cause a thing while they themselves do not do it directly.

Instinct means "inborn impulse." In the introduction Hahnemann says, "But the vital force, which of itself can only act according to the physical constitution of our organism, and is not guided by reason, knowledge and reflection."

Disorder first in Vital Force ! Why Do We Get Sick?

The material organism, without the vital force, is capable of no sensation, no function, no self-preservation; it derives all sensation and performs all the functions of life solely by means of the immaterial being (the vital force) which animates the material organism in health and in disease.

When a person falls ill, it is only this spiritual, self-acting (automatic) vital force, everywhere present in his organism, that is primarily deranged by the dynamic influence upon it of a Morbific agent inimical to life; it is only the vital force, deranged to such an abnormal state, that can furnish the organism with its disagreeable sensations and incline it to the irregular processes which we call disease; for, as a power invisible in itself, and only cognizable by its effects on the organism, its morbid derangement only makes itself known by the manifestation of disease in the sensations and functions of those parts of the organism exposed to the senses of the observer and physician, that is, by morbid symptoms, and in no other way can it make itself known.

It is clear that Hahnemann wishes to teach that it is a disorder of the activities of the internal man, a lack of harmony or lack of balance, which gives forth the signs and symptoms by which we recognize disease. These sensations constitute the language of disorder; i.e., the means by which we recognized disorder and disease. This immaterial vital principle, this simple substance, everywhere pervades the organism, it pervades every cell and every portion of the human economy. We will see in course of time that the change in form of a cell the result first of disorder, that the derangement of the immaterial vital principle is the very beginning of the disorder, and that with this beginning there are changes in sensation by which man may know this beginning, which occurs long before there is any visible change in the material substance of the body.

The patient himself can feel by his sensations the changes, and this is inimical to life, and death immediately follows, for life in tis fullest sense is freedom. As soon as the internal economy is deprived in any manner of its freedom, death is threatening; where freedom is lost death is sure to follow.

Disease products are comparatively harmless where it not for the fact that they contain an innermost and it is the innermost itself that is causative. The bacteria are the result of conditions within, they are, as it where, evolved by spontaneous generation - laterally, that is what is. Every virus is capable of assuming forms and shapes in ultimate. The causes of ultimate's are not form without but the immaterial invisible center. Those things that appear to man's eye are evolved, just as man himself is formed from a center which has the power of evolving, and endowment from the Creator, operating; under fixed general laws.

It is only when the vital principle is disturbed by cause of a disease character (that is the innermost of a virus in the form of a simple substance) that gives forth any consciousness of itself.

If there were no disturbing influence in the interiors of man he never would have symptoms. As you sit there in your seats in a perfect state of quietude or tranquility you are not conscious of your eyes, of your limbs, of your hair. You have to stop and think whether you feel or not. When all the functions are carried on in a perfectly orderly way you have no consciousness of your body, which means that you are in freedoms. When not in freedom the individual says, "I feel". It is this disturbance of an invisible character which comes form cause, and appears by changes in the activities of the body, changes in sensations, changes in functions. It is in accordance with all-wise providence that these sensations should appear to the physician who shall be intelligent enough to read them and know what they mean. They are a warning, they for us, for purpose. No feeling a man can have is without purpose, as there is nothing in the universe without its use. Hence these morbid sensations reveal to the physician that there is disorder.

To establish freedom should be the aim of the physician, and if a physician's work does not result in placing his patient in freedom he cannot heal the sick, for healing the sick in placing the patient in freedom, giving him absolute physical freedom....

Only the vital principle thus disturbed can give to the organism its abnormal sensations and incline it to the irregular actions we call disease.

This is totally different from calling the results of disease, e.g., calling Bright's disease, cancer or palsy diseases. Most of the conditions of the human economy that are called diseases in the books are not diseases, but results of disease. To call a group of symptoms a disease of none part, and another group of symptoms a disease of another part, is a great heresy and leads to errors in prescribing that can never be corrected. Organic change is the result of disease.

Morbid anatomy furnishes no basis for prescribing, but true pathology is often of the greatest benefit, helping the image of the sickness to shape itself before the mind.

What nonsense! That is not adjusting the remedy to the state of the patient that existed before he had these structural changes and fixed groups of symptoms, for if you adjust a remedy to the pathological condition you are not adjusting it to the patient, to his very beginnings down to the present time. He need not have pathological results. All he needs to have is symptoms. The patient can cure his own morbid anatomy. If you will take away the first state of disorder his economy will be safe. If the results of disease cannot be removed the patient himself will return to health and the morbid anatomy will undergo such changes that it will not affect his state of health. The fibrinous adhesions needs not necessarily go away a state of quiescence comes and remains year after year so long as he remains well.

To think of remedies for cancer is confusion, but to think of remedies for the patient who appears to have cancer is orderly, and you will be astonished to know what wonderful changes will take place in these conditions when remedies that correspond to the conditions before the cancer began are administered. Cancer is the result of disorder, which disorder must be turned in order and must be healed. We dwell upon this, for many paragraphs bring out this distinction between symptoms and results of disease. The true morbid sensations of a healthy organism are what we must first consider. It is first assumed that the organism is in a state of health and capable of performing its functions, and then the morbid sensations of this healthy organism are the symptoms that come to the physician as a forerunner of death in parts, and finally dealt of the whole. The patient tells the physician his sensations, of the numbness of his fingers, of the pricking in his skin, of the pain in his stomach, etc., all the sensations of any part of which he is reminded. The healthy man is not reminded of his parts. If he has pain or bleeding he is reminded of this part. If he passes his urine without sensation we say it is normal and he is in freedom, but if burning and smarting and tenesmus follow he is reminded of it, and these sensations constitute symptoms.

If the patient is waxy and pallid, he papules and pustules, or swollen and varicose veins with red face, red eyes, etc., these the physicians can see and not down. Again there are things that the physician cannot see and that the patient cannot tell, that the mother, sister, husband or wife should relate to the physician at his office.

These symptoms constitute what there is knowable of the sickness, that which appears to the mind of the physician upon which he makes up his verdict. When the strong symptoms are all gathered together the physician in studying the case must separate out those things that were observed years ago from those things that are observed today, nothing how they have changed and why changed. Sometimes they have been changed by drugs so that the whole nature of the economy is giving out a different group of symptoms.

Prescribing for the results of disease causes changes in the results of disease, but not in the sickness except to hurry its progress.

The physician must learn the changes all along the line, from beginning to end; what symptoms represented him now. Perhaps now he has morbid anatomy, pathological conditions in his lungs, liver and kidneys. The physician who has been for twenty years observing previous and present conditions in this manner, by hearing the symptoms can practically locate the morbid anatomy; he can tell where it will appear, knows where pus is in organs and where, and he can foretell pretty well what is soon to go on the economy.

Development and Manifestations of the True Disease "PSORA"

The word "psora" is derived from the Hebrew 'tsorat' and Greek 'psora' which means a groove or stigma. Psora is called the miasm of deficiency. Psora is rooted as a miasm in the disease of leprosy and is derived from skin eruptions of various types, including as scabies (itch), leprosy and psoriasis. Hahnemann determined that psora was spread through skin to skin contract in humans and also in animals. Psora starts as an itching eruption of the skin and is very contagious. Hahnemann discovered that a single touch or contact with clothing or bandages from the person infected with psora meant that person could contact the disease.

All babies are exposed to the transfer of psora during pregnancy and the birth process from the mother. After contact, psora is immediately transmitted by the nervous system throughout the entire body because it is spread via the skin. Every human being will have psora and it is considered to be the "mother" of all chronic disease. Since all human beings will have psora, it is therefore responsible for the largest percentage of all chronic human disease. For this reason, Hahnemann referred to psora as a "hydra-headed monster".

• It produces functional disturbance in the organism.
• A Psoric patient is always afraid of dying but lives on for years.
• Psoric patient is intelligent, over-sensitive, apprehensive.
• Overassertive patients, all kinds of functional complaints when arising from emotional disturbances.
• Sensitive to noise, to light and to odors.
• Patients are usually chilly, dirty and easily fatigued and depressed.
• Itching, skin dry, unwashed appearing with numerous eruptions.
• Itching by scratching, but skin burns and smarts afterwards.
• Hands and feet burn, flushes of heat, especially over face.
• Vertigo.
• Headache is relieved by lying down.
• Always hungry after eating; a bloating sensation that is sensitive to touch.
• Desires hot foods and meats.
• Constipation.
• Morning Diarrhoea.
• Cannot stat sill, the patient must walk or lean against something if he is on his feet.
• Susceptible to cold.
• All complaints are relieved by heat and in summer.
• All complaints are aggravated in winter.

Latent Psora

Latent Psora is a state when it remains slumbering or dormant without manifesting much disturbance and it has not yet come to the full outbreak of a manifest disease. Anyone who does not know the signs of its latent presence, would suppose and declare such persons to be healthy and free from any internal malady. But when such persons are exposed to some adverse circumstances the latent psora awakens and shows itself through varies types of manifest diseases.

Symptoms of Latest Psora

• Mentally alert and quick; active in his motions but easily fatigued both mentally and physically.
• Fatigue causes a desire to lie down and a dread to undertake anything requiring mental and physical exertion.
• Depressed and not accustomed to silent suffering.
• Cannot concentrate - subject to fits of anger.
• He worries and fears something.
• > by crying or natural eliminative functions.

Development of latent Psora

When the primary manifestations of psora (vesicular eruptions associated with intolerable voluptuous tickling itching and peculiar odour) are not cured by anti-Psoric treatment and are suppressed by local applications (artificial or natural), then the local cutaneous symptoms disappear but the disease is driven inwards and may remain dormant. This state is called as the latent state of Psora. In this state the disturbance is located in the central life mechanism, which is manifested through the perceptible sensorial and functional changes of the organism as a whole. Here nosology fails to be applied as the symptoms do not refer to any particular organ or tissue and the man through showing deviations from the healthy state is not termed as specifically diseased.

Why Tubercular Diathesis is given special attention?

The Psora, a most ancient miasmatic disease, in propagating itself for many thousands of years through several millions of human organisms of which each one had its own peculiar constitution and was exposed to very varied influences, was able to modify itself to such a degree as to cause that incredible variety of ailments which we see in the innumerable chronic patients, with whom the external symptom (which acts vicariously for the internal malady), i.e. the more or less extensive eruption of itch, has been driven away from the skin by a fatal art, or in whom it has disappeared of itself from the skin through some other violent incident.

The fact that this extremely ancient infecting agent has gradually passed, in some hundreds of generations, through many millions of human organisms and has thus attained an incredible development, renders it in some measure conceivable how it can now display such innumerable morbid forms in the great family of mankind, particularly forms in the great family of mankind, particularly when we consider what a number of circumstances contribute to the production of these great varieties of chronic disease (secondary symptoms of psora), besides the indescribable diversity of men in respect of their congenital corporeal constitutions, so that it is no wonder if such a variety of injurious agencies, acting from within and from without and sometimes continually, on such a variety of organisms permeated with the Psoric miasm, should produce an innumerable variety of defects, injured, derangements and sufferings, which have hitherto been treated of in the old pathological works, under a number of special names, as disease of an independent character.

Had psora never been established as a miasm upon the human race, the other two chronic diseases would have been impossible, and susceptibility to acute disease would have been impossible. All the diseases of man are build upon psora; hence it it the foundation of sickness; all other sickness came afterwards.

From a small beginning with wonderful progress, psora spreads out into its underlying states and manifests itself in the large portion of the chronic disease upon the human race. It embraces epilepsy, insanity, the malignant diseases, tumors, ulcers, catarrhs, and a great proportion of the eruptions. It progress from simple states to the very highest degree of complexity, not always alone and by itself, but often by the villainous aid of drugging during generation after generation; for the physician has endeavored with all his power to drive it from the surface, and has thereby caused it to root itself deeper, to become more dense and invisible, until the human race is almost threatened with extinction.

Look at the number of the population upon the face of the earth, and notice how few arrive at the age of maturity. When Tubercular diathesis will be "Predominant"

Tubercular diathesis is form in combination with Psoric and Syphilitic miasm. Tubercular diathesis will predominant if the following pathological indications are present in Family history, Past history and Present Complaints.

Diathesis: The constitutional or inborn state disposing to a disease, group of disease or metabolic or structural anomaly.

With the growth in knowledge of the Bacteriological science it becomes evident that practically all the disease known to be du to tubercle - bacillus infection are attributed by Hahnemann to Psora. Stuart close is of opinion that the causative agent is identical, and the two terms e.g., Psora and Tuberculosis are synonymous. The modern list is growing slowly by addition, from time to time. of other disease found to be pathologically or bacteriologically related to tuberculosis. it is quite possible that large part, if not all of the remainder of Hahnemann's list may ultimately be included in the modern list. It is another striking fact that Hahnemann chose Leprosy as the typical form of the ancient protein disease which he named Psora.

Modern Bacteriology finds that the bacilli of Leprosy resemble the tubercle bacilli in form, size and staining reactions and that the leper reacts to the tuberculin test and Wasserman Reaction test.

Mc Conkey, through clinical experience, came to believe and through that heart disease with or without valvular lesions, diabetes, rheumatism and cancer are tubercular in nature and origin.

Stuart close likes to fix the denotation of the Psoric miasm and identifies it with the Koch's tubercles bacillus. But we think that the time has not yet come to close the debate. Tubercle bacilli's are known to produce skin lesions but it is not yet experimentally verified or clinically demonstrated that they produce vesicular itching eruptions similar to those which are supposed to be produced by Hahnemann's Psoric miasm.
